OOC: Well, I've said nothing for quite a while Rosie but I'm gonna toss out some free advice here. Setting up a plot is fine and all, but to be blunt the questionnaire is taking planning things out way too far. We're a pretty spontaneous bunch by and large, most plots that plan things out too much just don't get off the ground. And setting roles ahead of time seems super overkill.
Besides that, the whole "3 strikes from the GM and you die" idea is absurd, the concept shows a desire to have a frankly insane degree of control over everything going on. And yes a GM is in control of things, except the players that is, I don't GM a ton, but I can say that when I do? I've always aimed to give the players something to well, play in. That means to no small extent, letting them do their own thing without trying to make sure everything goes one way. That's what all the best threads I've been in have done too, on the whole this thing seems to scream of wanting to be in control not just of yourself or your plot, but everyone around you.
And beyond all of that, you seem to have a habit of wanting to be in control of things in other threads. Bumping threads that aren't your own, and even pulling info on plots which you aren't running out. On the whole, your whole bit comes off very much as god moding at times. Most people I wouldn't bother to offer my thoughts, but honestly your writing itself actually is pretty alright. You have potential, but you need to let others run their own characters.
