Well, in the story, he died after saying that. So, either "going outside" means exiting ones body, and going outside to the next plane, or something else.

True, it did end with him dying.

Though if you think about it, the Palaces (and Sins) are like the journey described in 3301. And over the course of the game, the Phantom Thieve's and their reality was shaped.

They journeyed deep within Mementos, and at the end, they discovered that god had abandoned them.

Change of heart could mean one of two things. Either someone's heart will change to become Vain and that's the target, or it means the target is someone who's heart already is vain and would be changed as a result of removing it.
