Personally, I'd make an Abyssal entity which toys with the idea of "Distance is a Lie." Some form of creature which by existing make a directions fail to properly work, imagine taking a left yet ending up where you should have if you went backwards. Imagine not being able to explain any directions to anyone, because no matter what you say they'll always come out wrong. A massive glitch which messes up the very concept of movement and distance, that'd be pretty likely to cause chaos I think.

Prolly a creature with directable Anti-Magic capabilities and the ability to use multiple extremely dangerous spells. Make it intelligent so that it knows how to combat the enemy and outwit them, but so egotistical that it's easy to control through subtle manipulation of it's vices. Make sure to cover it's physical weaknesses by giving it a tough outer shell and multiple "arms" each with a different weapon attached to it. Now throw ALL that away. Make something that spams healing spells on itself constantly. Perfect enemy.
